2025 MCAS-Alt Score Appeal Form
If a school or district believes that a discrepancy exists between the score reported for a particular strand and the evidence submitted, the teacher who compiled the assessment and the school or program administrator must complete this online form and sumbit by 5:00 p.m. on June 20, 2025.

Appeal Process

A. Thoroughly review the following before completing this form:
B. Complete a Score Appeal Form for each strand being appealed only in one or more of the following rubric areas:
  • Level of Complexity,
  • Demonstration of Skills and Concepts, and/or
  • Independence.
(Note: Scores for Self-Evaluation and Generalized Performance may not be appealed, since these scores do not affect the overall performance level in the content area.)
For score appeals, please note that the Department will not accept:
  • submission of additional evidence;
  • requests to alter information previously submitted, or
  • claims that a clerical error was made in the creation of a strand.
C.  After submission of your appeal, print the on-screen confirmation you receive for your records. Original submitted assessment materials will be used by the Department of Elementary and Secondary Education as the basis of the score appeal review.
